Job Summary:
An Acute Rehabilitation / Transitional Care Unit (TCU) Registered Nurse (Acute Rehab / TCU RN) provides specialized nursing care to patients recovering from acute illnesses, surgeries, or injuries in an intensive rehabilitation setting. The Acute Rehab / TCU RN focuses on managing complex care needs, promoting recovery, and facilitating the transition of patients from acute care to home or lower levels of care, ensuring optimal outcomes.
Key Responsibilities:
- Assess, plan, and provide nursing care to patients in acute rehabilitation and transitional care units recovering from illnesses, surgeries, or injuries.
- Collaborate with multidisciplinary teams, including therapists, social workers, case managers, and physicians, to develop and implement individualized care plans.
- Monitor and manage patients’ vital signs, pain levels, mobility, and progress toward rehabilitation goals.
- Administer medications, IV therapies, wound care, and other treatments related to the recovery process.
- Provide education and support to patients and families regarding care plans, medication management, wound care, physical therapy, and self-care strategies.
- Assist patients with daily activities such as mobility, transfers, and personal care, promoting independence and safety.
- Recognize and respond to potential complications such as infections, pain, or mobility issues, implementing interventions as needed.
- Document patient assessments, interventions, and outcomes accurately and in compliance with hospital policies and regulatory standards.
- Facilitate patient transitions from acute care settings to home or outpatient services, collaborating with case managers, social workers, and other team members.
- Participate in interdisciplinary team meetings, contributing input on patient progress, goals, and discharge planning.
Qualifications:
- Education: Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) accepted.
- Licensure:
- Valid Registered Nurse (RN) license in the state of practice.
- Certification:
- Basic Life Support (BLS) certification required.
- Advanced Cardiac Life Support (ACLS) and/or Certified Rehabilitation Registered Nurse (CRRN) preferred.
