Job Summary:

n

A Long-Term Care (LTC) Registered Nurse (RN) provides comprehensive, patient-centered care to individuals in long-term care facilities, such as nursing homes, rehabilitation centers, and assisted living communities. They are responsible for managing the overall health and well-being of patients, ensuring personalized care plans, and collaborating with interdisciplinary teams to promote optimal outcomes.

Key Responsibilities:

    n

  • Provide direct nursing care to residents in accordance with individualized care plans.
  • n

  • Administer medications, perform treatments, and monitor patients’ progress, ensuring safety and adherence to established protocols.
  • n

  • Assess, plan, implement, and evaluate nursing care for long-term care residents.
  • n

  • Collaborate with physicians, therapists, and other healthcare professionals to develop and adjust patient care plans.
  • n

  • Monitor and document residents’ health status, including vital signs, medication administration, and overall progress.
  • n

  • Educate residents and their families on health conditions, care needs, and treatment options.
  • n

  • Respond to and manage acute and chronic conditions, ensuring timely interventions and care coordination.
  • n

  • Provide emotional support and counseling to residents and their families, promoting a positive living environment.
  • n

  • Ensure compliance with all federal, state, and facility regulations related to long-term care.
  • n

  • Supervise and delegate tasks to licensed practical nurses (LPNs) and nursing assistants (CNAs).
  • n

  • Participate in quality improvement initiatives, staff training, and continuing education.
  • n

  • Maintain accurate patient records and documentation, adhering to established clinical standards and protocols.
  • n

  • Address concerns related to infection control, safety, and preventative care to minimize risks to residents.
  • n

Qualifications:

n

    n

  • Education: Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) accepted.
  • n

  • Licensure:n
      n

    • Valid Registered Nurse (RN) license in the state of practice.
    • n

    n

  • n

  • Certification:n
      n

    • Basic Life Support (BLS) certification required.
    • n

    • Advanced certifications such as ACLS or Geriatric Nursing (e.g., CGRN) preferred.
    • n

    n

  • n

  • Experience: Previous experience in long-term care, gerontology, or related setting
  • n
