Education Verification - required for all first time Wellspring Travelers

Physical - Clearance confirming capability to perform job duties 

Including but not limited to MMR, HepB, TDap, and Varicella

*All records are subject to specific assignment needs.  If you’re missing anything, your Credentialing Specialist will help to set you up to obtain any necessary records.

• Provide your most up to date resume.  Make sure to detail any work history gaps even if you’re noting that you had 1 month of personal time off.
• Use your legal name on all documents.
• If your name does not match on all documents due to a name change, provide the relative legal document.
• Prepare copies of your basic government-issued ID’s (driver’s license, passport, social security card, etc).
• Prepare any and all health certifications as soon as possible.
• Extra certs?  Send them along too!  Our team will help you identify which documents are required.
• Let your credentialing specialist know your preferred method of communication.
• Be responsive, even if you’re letting them know that you’re working and can’t get to it right away.  Over communication is better so we can manage expectations all around.
• Notify your credentialing specialist if you’re planning to travel anytime between offer acceptance and your start date.
• Start building out your profile and attaching your documents in our online portal to get a head start!
