Job Title: Hospice Registered Nurse (RN)**
Job Summary:
A Hospice Registered Nurse (RN) provides compassionate, patient-centered care to patients with terminal illnesses and their families, focusing on improving quality of life, managing symptoms, and providing emotional and spiritual support. The Hospice RN works in a multidisciplinary team to ensure comfort, dignity, and support during the end-of-life journey.
Key Responsibilities:
- Provide skilled nursing care to patients with terminal illnesses, focusing on pain management, symptom control, and emotional support.
- Develop individualized care plans to address physical, emotional, and psychosocial needs, ensuring patient comfort and dignity.
- Administer medications, including pain relief, symptom management, and hospice-related treatments.
- Educate patients and families about hospice care, prognosis, treatment options, and emotional support.
- Collaborate with physicians, social workers, chaplains, counselors, and other healthcare professionals to provide holistic care.
- Monitor and assess patients for changes in health status, adjusting care plans as necessary.
- Provide emotional, spiritual, and psychological support to patients and their families, including end-of-life counseling.
- Offer bereavement support to families after the patient’s death.
- Ensure patients and families understand their options for care, including advance directives, palliative care, and end-of-life planning.
- Document patient care accurately, including symptoms, treatments, and progress, in compliance with hospice care standards.
- Provide education and support to caregivers and family members on caring for the patient in the home setting.
- Promote infection control, safety, and dignity in end-of-life care settings.
Qualifications:
- Education: Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) accepted.
- Licensure:
- Valid Registered Nurse (RN) license in the state of practice.
- Certification:
- Basic Life Support (BLS) certification required.
- Hospice and Palliative Care Certification (CHPN) preferred.
- Experience: Previous experience in hospice care, palliative care, or related healthcare settings
