Job Title: Skilled Nursing Facility (SNF) Registered Nurse (SNF RN)**

Job Summary:

A Skilled Nursing Facility (SNF) Registered Nurse (SNF RN) provides comprehensive nursing care to residents in long-term care or rehabilitation facilities. The SNF RN is responsible for assessing, planning, implementing, and evaluating individualized care plans to promote the health and well-being of residents. They ensure compliance with regulatory standards while supporting patients recovering from acute illness, surgery, or chronic conditions.


Key Responsibilities:

  • Assess, plan, and deliver nursing care to residents with complex medical needs, chronic illnesses, or post-acute conditions in a skilled nursing facility setting.
  • Collaborate with multidisciplinary teams, including physicians, therapists, social workers, and other caregivers, to develop and implement care plans.
  • Administer medications, treatments, and therapies such as wound care, pain management, and rehabilitation services.
  • Monitor residents’ health status, including vital signs, laboratory values, and overall progress, identifying potential complications and implementing interventions as needed.
  • Provide education and support to residents and their families regarding treatment plans, medication management, and self-care strategies.
  • Coordinate and facilitate discharge planning, including working with case managers, social workers, and other care team members to ensure a smooth transition to home or lower levels of care.
  • Document assessments, interventions, and outcomes, ensuring compliance with state and federal regulations and facility policies.
  • Monitor and maintain medical equipment, such as oxygen therapy, wound care supplies, and mobility aids.
  • Support patients with daily living activities such as bathing, dressing, mobility, and medication management.
  • Participate in quality improvement initiatives and staff education related to patient care, safety, and regulatory compliance.

Qualifications:

  • Education: Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) accepted.
  • Licensure:
    • Valid Registered Nurse (RN) license in the state of practice.
  • Certification:
    • Basic Life Support (BLS) certification required.
    • Advanced Cardiac Life Support (ACLS) and/or certification in geriatrics preferred.
