2 years inpatient discharge planning experience is required.
Ortho experience strongly preferred.
Occasional weekends and holidays.
May float to different units.

Beds: approximately 1,000 bed Level I Trauma hospital.

Caseload of approximately 16 to 24 cases per day.

EMR- Epic Case Management Module.
Attire: Business casual and white lab coat
Hospitalist or Intensivist in house 24/7
RT on site- 24/7

Pharmacy on site- 24/7
Shift and Weekend expectations (i.e. 6a-6p; Every other weekend- Floating required)

Holiday and on-call expectations (i.e. on call 3X per week; holiday – 1 major, 1 minor)
